I've been thinking about this a while. I thought I'd post my thoughts and see what you think.
The motto of BDSM is Safe, Sane, Consensual, but it seems like a lot of ballbusting studios strive for the exact opposite. There is no visible safety elements (safewords), the Dommes actually try to injure their subs, and any consensual elements are downplayed.
I'll cite Uk Mistress E**** as my primary example, which I think is fair since it is the #1 studio on Clips*****. She claims to have injured several subs, and has expressed amusement at doing so, and claims to actually be trying to do so.
So I have 3 questions:
1) Is this even real? Why go through the trouble (legal and otherwise) of injury someone if you can just claim you did?
2) Why? The answer is money, but why does this make money? Why do clips like this sell?
3) Is it ethical to even purchase these kinds of clips? Assuming they are real, they are pretty sick when you detach it from the fetish we all love.
What are your thoughts?

I would say that one may be able to make a good argument that some ballbusting videos are dangerously hardcore (insane) but there are definitely many videos that are much lighter and clearly more safe. I have very little personal experience with the general BDSM community outside of ballbusting specifically but I would say that most ballbusting play (amatuer and professional) is safe, sane, and consensual.
These are my answers to your three questions.
1) I have watched many of UK Mistress E's videos over a fairly long period of time (prior to her current ********** studio) and I would say that the ballbusting is definitely real, in the sense that she is definitely utilizing considerable force. I doubt that she is truly intending to injury her subs because many of her shots seem intentionally indirect (hitting their ass with most of the force). However, she may very well be completely serious and may wish to injure her subs. There may be safe words that are involved along with fake safe words that are used so that the viewers think that the mistress is more sadistic then she really is. I can definitely tell you that many people are turned on by the sadism of a ballbusting dom and so it is generally more marketable for these Doms to pretend that they wish to intentionally injure their subs. Signing legal waivers help to mitigate the chance of legal liability but generally any sub that would consent to such an arrangement is not likely to resort to a lawsuit if something goes wrong.
2) The reason why these hardcore ballbusting videos make money is because it is extremely thrilling to many ballbusting enthusiasts to see a Dom who at least appears to be completely sadistic. A truly sadistic dom would love or at least would not particularly be concerned about injuring a sub. Many of the people who buy these type of videos want the action and the sadism of the dom to be as real as possible and Mistress E has a financial interest in making it seem that she is as Sadistic as possible. Not that it matters but I do not like to watch her videos and I am not a masochist at the level of many people who enjoy ballbusting.
3) It is certainly legal to purchase these videos but it is definitely arguable as to whether it is ethical for one to purchase them. I would not purchase any of her videos anymore because it is pretty sick and disturbing to me. By purchasing videos like Mistress E's one could argue that a customer is indirectly contributing to the exploitation of people with a dangerously unhealthy desire to be abused. If there was no money in videos that involved hardcore ballbusting that involves a mistress intending to injure her subs then there would be less doms engaging in that behavior. This would make is more difficult for sub guys to endanger their health with their dangerous desire to be injured during ballbusting. Some guys out there are a serious danger to themselves and need help because they want to be injured and a mistress may be willing to indulge their fantasies because there is extra money in truly hardcore ballbusting videos.
On another note I wish there was more sensual ballbusting and or ballbusting videos that did feel so fake and scripted. I hate it when a ballbusting videos feels likely a cheesy porn video. There is some good amateur ballbusting videos out there but not very many and most of those are too soft for my taste.

Many of us in the bdsm lifestyles have switched from the SSC model to the RACK (rish aware consensual kink) just because what we do isnt really safe or sane. But it is consensual between both partners that there is a risk, and that we are aware and agreeing to it.

Personally, I do try to play carefully, but were I injured by a domme, I'd hope she would look after me. That being a given, we'd have two choices to make:
1. to sit around feeling miserable about the injury and wallow in guilt and shame forever.
2. being a pair of a sadist and a masochist, poke fun at the experience and make what we can of it.
As long as I trusted the domme, I'd much prefer option number 2. As long as something was learned of course.
This in no way justifies any scenario where a sub is abused by taking him or her past their limits without their consent (including by coercion). That would be very wrong (and I suppose there are well, dommes and Doms out there who are abusive). I'm explaining why a video where an injury occurred doesn't necessarily freak me out or imply that exploitation is happening. Also I think it's responsible to say that an injury occurred, it's a disclaimer that says if you do this, you will likely be injured. Same with high-risk sports, people get into them consensually, and videos of injuries are common, willingly posted by the participants and shared with enthusiastic viewers. I'm not sure that makes it 100% clear ethically (by whatever standards) but there is a level of consent/awareness present and 'insanity' is not necessarily a factor. (Though I've wondered that after seeing a bloodied face of spaghetti after a boxing match.)
Also people take risks because they believe themselves immortal and sometimes learn the hard way. Last but not least, if everyone is completely aware of what it is they are doing and decide that they want to take the risk, it is well, their choice. There's always a risk with any porn I suppose that someone is being exploited. Personally what I look for in any bdsm porn, make that any porn, is signs that both parties are actively wanting to be there. If the mutual enjoyment and consent is evident, I'm far more open to the concept of RACK.
(RACK goes well after you've seen a guy racked and curled up on the ground in the fetal position, yet he's moaning with pleasure, or laughing).
I suppose there's a lot more things you could bring up here but for now I guess I'll just leave my 2 cents (err, nickel now, we no longer have pennies) at that.

I personally like a good acting where she hits him, he falls dramatically on the floor and she mocks him afterwards rather than a real hit.
I personally don't like to see someone get hit repeatedly and him just standing there and taking it.
From an ethical point of view, it's far worse to restrict people from doing what they like than letting him.
Edit: I don't think the dom goes over his limit, it may be him who wants her to talk like she does, but if it does happen, it's wrong and it should stop.
