Actually... it might have something to do with puberty. Like, when you're developing is when you form most of your associations that develop into kinks. When you're developing, you most likely are, in fact, a lot smaller than you'll end up -- and that's also the time when you're most curious how you compare, so if you do the comparison to an adult norm then, you'll undoubtedly fail to measure up, and that feeling of inadequacy (or at least fear about whether or not you will eventually be a normal size) might stick with you even after you're fully developed and most certainly not small after all.

For myself... apparently I'm large. I say apparently because I refused to believe it for a long time. Girls would always tell me I was big, but I figured it was just something that you say -- you're hardly going to tell a partner he's small, right? Eventually I realised that normal condoms were painfully tight and that I had to wear XLs... and eventually enough girls told me, and a couple guys friends who saw me naked (including one beautiful pansexual man who's 6'4 and muscular: we were at a play party together and he saw me fooling around with this girl, commented afterwards "has anyone ever told you you've got a big dick?").
