As somebody pointed out, these work better when there's some kind of story or motivation behind the image. To me, since I'm not into the CCC (cuckold, chastity, **********) stuff, they carry a charge when:
1) there's some kind of contest between the man (usually unseen) and the woman the pic focuses on. The contest can be boxing, wrestling, anything.
2) He bets her he can do something, or hold back from doing something, with his reward being sex. If he loses, she gets to hurt him in the genitals.
3) She of course knows how much the ball treatment will hurt him. She also knows of her sexual power, and she's relying on it to defeat him. Usually, in my vision, it's that he's been without sexual relief for quite a while, and she's going to use that against him.
4) If there's no contest or bet stated, then she's out for revenge -- perhaps he cheated on her, and doesn't know she knows. He thinks he's going to feel good. She plans a surprise: He's going to feel bad.
