You need to extract it with winrar or 7zip and then double-click on the install.exe. If you have windows 7 or vista, it's better to install it on a folder different than program files because that can cause permissions issues.

I figured that out. Now I keep getting stuck in the screen where it looks like she wants you to enter your name. I enter different things, click everywhere... the sound just keeps looping.
