Actually... it might have something to do with puberty. Like, when you're developing is when you form most of your associations that develop into kinks. When you're developing, you most likely are, in fact, a lot smaller than you'll end up -- and that's also the time when you're most curious how you compare, so if you do the comparison to an adult norm then, you'll undoubtedly fail to measure up, and that feeling of inadequacy (or at least fear about whether or not you will eventually be a normal size) might stick with you even after you're fully developed and most certainly not small after all.

For myself... apparently I'm large. I say apparently because I refused to believe it for a long time. Girls would always tell me I was big, but I figured it was just something that you say -- you're hardly going to tell a partner he's small, right?
